Assessment of Attendant Care Needs

An Attendant Care Needs Assessment is a detailed assessment of a client's personal care needs. Its goal is to determine the quantity and the complexity of assistance that the client will require with personal care in order to be able to function effectively in his/her own environment.

The assessment involves:

• Observing the client performing different aspects of their personal care
• Determining and prioritizing the physical limitations and deciding the type, intensity, and the frequency of the skilled need required

A detailed Form 1 will be completed which includes calculating the attendant care costs outlining the client's functional status and their care needs.
