Initial Needs Assessment
An Initial Needs Assessment determines what services and/or equipment a client may need in order to safely return home from hospital following an injury. This assessment is completed either in hospital or in the client's home prior to the client being discharged in order to facilitate a safe transition home.
The assessment involves:
- File review of client's medical history
- Physical assessment (strength, range of motion, tolerances)
- Home Functional Assessment
- Equipment needs assessment
- Discussions with treating health care team and family
Recommendations are made regarding equipment needs, the need for further occupational therapy intervention, the need for homemaking assistance and attendant care.
